These forms are for use by new clients who have made an appointment with me. Rather than coming in 15 or 20 minutes early to fill out the "new client" intake paperwork, clients can download the forms, print them, and fill them out at home! And then please don't forget to bring them with you when you come in! How to use these forms: simply click on the form you need, print and then close that browser window when finished (by clicking, you'll raise a separate browser window).
